  • When am I able to register on this site and complete the online PHA questionnaire? Please allow 5 business days to pass after your blood draw appointment before you begin the online questionnaire, which allows ThedaCare time to process and load your lab and biometric data.
  • Don’t know your “access code”? Your unique access code is shown on the PHA instruction sheet, provided by the employer sponsoring the PHAs. If you need to obtain this, contact your company’s Human Resources Department or Wellness specialist.
  • Are you a returning guest to our site, trying to do your PHA questionnaire, and don’t remember your Password from last year? Once you enter your registration information, your login and email should auto-populate on the next screen. You can simply enter a new password each year.
